3) Click Continue
4) Key in Your Name
5) Key in Your IC
6) Key in Present Address, Postcode, City and State.
7) Key in Body Temperature of The Day
8) Answer 2 Question
9) Tick "I argree that the above information provided is complete and true."
10) Click Submit

Reporting:
1) You can generate the health care data report by click Analysis menu and click on Health Care Report.
