Due to the ongoing and rapidly changing situation with the coronavirus disease 2019 (COVID-19), we are requiring all customers to fill out the Health Declaration Form below. We are collecting and processing this personal data for the necessity of protecting against serious many cross-border threats to the health. We do this as preventive measures for our workforce and customers to mitigate the risk of COVID-19 outbreak in our company. This form will be stored in our medical and security team and is not shared with any third party unless there will be an official request by the local authorities for reasons of public interest in the area of public health.
The form will be deleted one month after collection unless a longer period is required by the public authorities.
